As Yaşam Diyet, acting in the capacity of data controller pursuant to the Turkish Personal Data Protection Law No. 6698 (KVKK), we exercise the utmost care in the processing and protection of personal data belonging to our clients. This disclosure notice contains details regarding the processing of your personal data that you have provided to us in your capacity as a client.
Belonging to our clients, identity information such as name, surname, Turkish Republic ID number, date of birth, and gender; contact information such as mobile phone number, e-mail address, and residential address; health and nutrition data such as height, weight, body mass index, body analysis, health history, medications used, nutritional habits, and allergy information; appointment information such as appointment dates and times and attendance status; and payment information are processed.
Your personal data are processed for the purposes of carrying out the client registration process, providing dietitian services, preparing personalized nutrition programs, monitoring and analyzing your health status, planning appointments and sending reminders, carrying out payment transactions, measuring client satisfaction, providing information on health and nutrition matters, and fulfilling legal obligations.
Your personal data are collected through face-to-face meetings during your applications to our center, telephone calls, e-mail correspondence, forms submitted via our website, and WhatsApp messages. These data are processed based on the legal grounds set out in paragraph (2) of Article 5 of the Turkish Personal Data Protection Law No. 6698 (KVKK), namely that processing is "directly related to the establishment or performance of a contract", that it is "necessary for the data controller to fulfil its legal obligation", and that it is "necessary for the legitimate interests of the data controller, provided that the fundamental rights and freedoms of the data subject are not harmed". Your health data are processed pursuant to Article 6 of the Turkish Personal Data Protection Law No. 6698 (KVKK) and the relevant regulations.
Your personal data may be transferred, in accordance with the procedures and principles set out in Articles 8 and 9 of the Turkish Personal Data Protection Law No. 6698 (KVKK), to authorized public institutions and organizations for the fulfilment of our statutory obligations; to laboratories and diagnostic centers with which we cooperate for the coordination of healthcare services; to banks for the execution of payment transactions; to attorneys from whom legal consultancy services are obtained; and, where necessary, to insurance companies.
Pursuant to Article 11 of the Turkish Personal Data Protection Law No. 6698 (KVKK), you have the right to learn whether your personal data are being processed; to learn the purpose of processing and whether they are used in accordance with that purpose; to know the third parties to whom they have been transferred domestically or abroad; to request the rectification of incompletely or inaccurately processed data; to request the erasure or destruction of data within the framework of the conditions foreseen in Article 7 of the Turkish Personal Data Protection Law No. 6698 (KVKK); to request that third parties to whom the data have been transferred be notified accordingly; to object to the emergence of a result against you through the analysis of processed data exclusively by automated systems; and, in the event you suffer damage due to the unlawful processing of data, to request compensation for such damage.
